THEY HAVE TO SAY)The Inpatient Unit CNA works on a collaborative
interdisciplinary team including Social Workers, RNs, Volunteer
Coordinators, Grief counselors, and Clinical Assistants."I have

patients." - Inpatient Unit EmployeeHOW YOULL MAKE A DIFFERENCE IN
THIS ROLE (ESSENTIAL FUNCTIONS)The CNA IPU is responsible for
patient and family satisfaction by assisting with activities of
daily living, providing light homemaker help, following the plan of
care, communicating changes in condition, and documenting care. The
CNA IPU provides patient and family centered care consistent with
Agrace philosophy, goals, standards, regulations and
policies.Assist Patients with Activities of Daily LivingAssist with
ambulation, changing position, and moving to/from sitting and
standing.Help with bathroom functions like toileting and
incontinence care.Help bathe, shower, dress, provide oral care, and
groom.Assist with eating and drinking.Perform post mortem
care.Obtain vital signs. Assist with treatments.Anticipate and
respond to patient needs and round on patients.Provide homemaker
assistancePerform light housekeeping such as changing linens, doing
laundry, tidying room, and removing trash.Deliver patient
supplies.Deliver food and beverages.CommunicationRead, understand,
and follow the plan of care. Document cares and changes in patient
condition in electronic health record.Document and report
significant changes in condition or unusual incidents to
appropriate Interdisciplinary Team Member such as client
abuse/neglect, suicidal intent, harmful coping mechanisms or
violence, etc.Reinforce patient and family
education.MiscellaneousParticipate in meetings to plan and evaluate
patient/family services.Precept new staff and students.Delegate
tasks to volunteers as appropriate.YOU BRING (EXPERIENCE &
QUALIFICATIONS)High school diploma or GED, or equivalent
combination of education and work experienceGraduation from an
accredited Certified Nursing Assistant programCurrent, unrestricted
Nursing Assistant Certification in state of employmentOne-year
personal care experience, hospice, or home health, preferredMust be
at least 18 years of ageAbility to develop, maintain, and apply
knowledge of hospice and end of life processAbility to maintain
professional boundaries with patients, families and
co-workersAbility to follow plans of care, and patient, family, and
clinical staff directionsBasic Microsoft Office skillsExperience
using electronic health records; Epic preferredAbility to work
effectively and cooperatively with othersAbility to communicate
effectively verbally and in writingAbility to be flexible in time
and task to meet organizational needsAbility to maintain
confidentiality about patients, families, caregivers, staff,
donors, and organizational intellectual propertyAbility to use
proper body mechanics, safe patient handling, and standard
